A Case of Devic's Disease

چکیده

A man, aged 48 years, who is by profession a sorter in Railway, was admitted on 21-8-51 at 6 p.m. with a complaint of inability to walk, blindness and incontinence since 7 a.m., 20-8-51. On 12-8-51 he had fever with hiccoups which lasted for a day and he was not unconscious during the fever. On 17-8-51 he developed numbness of the lower limbs and retention of urine which was relieved by catheterisation by a private practitioner. On 19-8-51 he developed severe pain localised in the sacral region which lasted only for a day. On 20-8-51 at about 7 a,m. when he got up from his bed, he found that his lower limbs were paralysed and at the same time, he had lost vision in both, the eyes. He was admitted into Pentland Ward on 21-8-51 at 6 p.m. after having been catheterised in the casualty. He was having constipation also. He did not suffer from any previous illness and he denied history of veneral exposure. Patient is aged about 50 years, fairly well nourished; regional glands not enlarged; intellectual functions normal; speech normal; there is complete lote of vision. Pupils dilated not reacting to light. All other cranial nerves are normal. Motor power is normal in upper limbs but completely lost in both the lower limbs. Tone is normal in upper limbs but flaccid in lower limbs. No wasting, no abnormal movements. Sensations in the upper limbs are normal but all sensations are lost in lower limbs and the abdomen below a level 1" above the umbilicus. (8 T) All reflexes are normal in the upper limbs.
